Enrollment forms will be handed out on Monday, May 3, 2011 beginning at 9:00 a.m. to those who have met all the requirements, proof of residency, proof of immunizations, and proof of age. No enrollment forms will be handed out before May 3, 2011 or to those without proof of residency.

We require an Immunization Card. This is a special card that contains a list of all of the immunizations your child has received. When fully folded, the California version tends to look like the following:
